What Is Gynecomastia?

Gynecomastia is enlargement of the male chest caused by glandular tissue, fat, excess skin or a combination of these factors.

It can affect one or both sides and may become noticeable during puberty or adulthood. Some men experience tenderness, prominent areolae or chest fullness that remains despite weight management and exercise.

Clinical assessment helps identify whether the enlargement is mainly caused by gland, fat, loose skin or another contributing medical factor.

Clinical guidance: Not every enlarged male chest requires surgery. Suitability should be decided only after history, examination and appropriate assessment.

Grade 1, Grade 2 and Grade 3 Gynecomastia

The surgical plan depends on gland size, fat distribution, skin quality, chest symmetry and areola position.

Grade 1

Mild Enlargement

A smaller amount of tissue beneath the areola, generally with little or no excess skin.

Grade 2

Moderate Enlargement

More visible glandular tissue or fat that may extend beyond the areola.

Grade 3

Advanced Enlargement

Greater chest volume, potentially accompanied by loose skin, sagging or lower areola position.
