What Is Gynecomastia?
Gynecomastia is enlargement of the male chest caused by glandular tissue, fat, excess skin or a combination of these factors.
It can affect one or both sides and may become noticeable during puberty or adulthood. Some men experience tenderness, prominent areolae or chest fullness that remains despite weight management and exercise.
Clinical assessment helps identify whether the enlargement is mainly caused by gland, fat, loose skin or another contributing medical factor.
Common concerns may include:
- Firm tissue beneath the areola
- Localised fat around the chest
- Puffy or enlarged areola
- Unequal enlargement on one or both sides
- Loose skin in more advanced cases
- Chest fullness despite exercise

Grade 1, Grade 2 and Grade 3 Gynecomastia
The surgical plan depends on gland size, fat distribution, skin quality, chest symmetry and areola position.
Mild Enlargement
A smaller amount of tissue beneath the areola, generally with little or no excess skin.
Moderate Enlargement
More visible glandular tissue or fat that may extend beyond the areola.
Advanced Enlargement
Greater chest volume, potentially accompanied by loose skin, sagging or lower areola position.
How Gynecomastia Surgery May Be Planned
The technique is selected according to the underlying tissue and grade rather than applying one method to every patient.
Liposuction
May be considered when localised fat contributes significantly to the chest enlargement.
Gland Excision
Dense glandular tissue beneath the areola may require removal through a carefully planned incision.
Combined Treatment
Some patients require both liposuction and gland removal to address different components of chest enlargement.
Skin Adjustment
More advanced cases with significant excess skin may require a separate skin-management strategy.
Exact technique, incision placement, anaesthesia and surgical facility are decided only after clinical evaluation.

Gynecomastia Surgery FAQs
These answers provide general information. Personal advice requires examination and medical assessment.
Ask a QuestionExercise may reduce general body fat and improve chest muscle shape, but it does not directly remove firm glandular tissue. Assessment helps distinguish fat-dominant fullness from true glandular gynecomastia.
It depends on tissue composition. Liposuction may address fat, while dense glandular tissue may require excision. Some patients need both.
Every incision creates a scar. Placement and length depend on the technique and grade, and scar behaviour varies among individuals.
Cost depends on grade, technique, anaesthesia, facility, investigations and individual treatment requirements. A personalised estimate is provided after assessment.
Return to work and exercise varies according to procedure extent, type of work and healing. Individual instructions are provided during follow-up.
Recurrence risk depends on factors including hormonal changes, medication, steroid use, weight fluctuation and the underlying cause.
Yes. Gynecomastia evaluation is available at Kimaya Clinic by appointment. Evening OPD is generally Monday to Friday from 6 PM to 8 PM.
